The Survey Center allows you to create custom surveys and questionnaires and deliver the questionnaires (to you or vendors) during site visits, contract reviews, or employee interviews.  Surveys may also solicit information from the vendor community; such as outreach events or disparity study questionnaires.  The results of the surveys are stored in the database for reporting and/or trend analysis.  Once a survey is created it can be used many times.

Surveys are a series of questions that are displayed when the survey is started.  You create the survey by entering the questions, selecting the acceptable response types, and selecting the delivery method.  There are a few other details but they are negligible.

Surveys Types

Before you can do anything, Question Sets (Surveys) must be created and questions added to the question sets.  This is done in the PRISM Survey Center Configuration.  Depending on what type of Question Set/Surveys you configured will determine when  and where your surveys will appear in PRISM.  Surveys are segmented by Question Sets.  Question Sets have a type (Site Visit, Employee Interview, Custom).  The Type determines where the site visit can be delivered to users.

Survey Delivery

Surveys are delivered to you or vendors.  Based on the Survey Type and the selected Delivery, the survey may:

Be displayed to you at will.  This is the case when the survey is a site visit or employee interview

Be displayed to the user after login or upon entering a module.  This is the case when a surrey's delivery is custom
