Creating a Survey

Creating a survey is trivial.  There are three parts to creating a survey; setup, adding questions, and selecting the delivery.

 

 

 

Creating a Survey

Before creating the survey in PRISM you should already have your survey questions and survey name.

 

Naming and Survey Type

 

1From the Survey Center survey list, enter the survey name in the Title column of the list of surveys (the last row of the list)

2Select the Survey Type (Site Visit or Employee Interview or Custom). 

3If the Survey Type is Site Visit or Employee Interview, select the Scope of Use (module) the survey should be available

4Set the Visibility to "N" (this will make the survey invisible while you are creating it)

5Click "Add New Question Set" link  (The new survey is created but it has no questions and no delivery type.

 

Adding Questions to the Survey

 

1From the Survey Center survey list, identify the survey added above, click the Title link

2Click the "+Add Question" button

3From the Question Details section, enter the survey Question

4Is there a comment? Affirm "Allow Comment"  Enter Comment label

5Enter the Response Type*

6Enter the Sort order of the survey question (each question should have a unique number)

7Repeat for all of the survey questions

 

 

Note: If a Response Type of "List" is selected, enter the discrete list items in the text box.  Separate each item by a semi-colon.

 

Scope of Use and/or Delivery

The survey delivery depends on the survey type.

1From the Survey Center survey list, identify the survey added above, click the "Update" link

2If the survey type is "Custom", click the "Customize Delivery" link

 

 

3Affirm "Enable Survey".  This will send invitations to complete the survey to the target audience when the delivery is saved.  Uncheck this parameter until you are sure you are ready to send the invitation.

4Select the Delivery. "At Logon" will delivery the survey after the vendor logs into PRISM.  "At Entry Of:" will delivery the survey when the vendor enters the selected module"

5Affirm "Required".  This determines if the vendor is required to take the survey or the survey is optional.  if required, the vendor will not be able to do anything until the survey is completed.  otherwise, the vendor may ignore the survey and continue using PRISM.

6"Select an "Audience".  The audience is the collection of vendors that the survey will be delivered.  If "--All --" is selected, the survey will be delivered to all of the vendors in PRISM.  Otherwise an Advance Search may be selected.  The vendors that are part of the advance search results will receive the survey after login.  Moreover, the vendors will receive an email indicating they should log into PRISM and take the survey.

7Enter the "Survey Introduction".  This introduction will be displayed to the vendor before the survey starts.  It should inform the vendor how much time the survey will take and the purpose of the survey.

8Click "Save Delivery".  If "Enabled" is affirmed, the invitations to complete the survey will be sent to the vendors immediately.

 

 

Finally, from the survey list, update the survey and make the survey visible.
